Αν η επιδερμίδα εκτίθεται σε μόνιμη πίεση και τριβή, αυξάνεται η κυκλοφορία του αίματος με αποτέλεσμα την αυξημένη ανάπτυξη κυττάρων στο συγκεκριμένο σημείο. Σχηματίζεται λοιπόν μια στιβάδα σκληρού δέρματος (σκλήρυνση), η οποία μεγαλώνει εσωτερικά σε σχήμα κώνου και δημιουργείται ένας σκληρός πυρήνας, ο οποίος ονομάζεται ρίζα του κάλλου. Ο κάλλος προκαλεί έντονο πόνο ο οποίος δημιουργείτε από την πίεση του πυρήνα στα νεύρα που διασχίζουν τις κατώτερες στοιβάδες του δέρματος και μπορεί να είναι τόσο επώδυνος ώστε να επηρεάζουν το βάδισμα, την επιλογή υποδημάτων αλλά και τις δραστηριότητες του ατόμου. Οι κάλοι και οι σκληρύνσεις είναι αποτέλεσμα πολλών παραγόντων : • Εξωγενή (στενά υποδήματα, ανοικτά υποδήματα, ανωμαλίες στα υποδήματα καθώς και αθλητικές δραστηριότητες) • Ενδογενή (οστική προβολή, κάταγμα σε κακή θέση) καθώς και • Διαταραγμένη μηχανική του ποδιού (κοιλοποδία, παραμορφώσεις δακτύλων, κοντό 1ο μετατάρσιο, hallux rigidus, βλάβες από μεταφορά λόγω οστεοτομίας ή αφαίρεσης κεφαλής διπλανού μεταταρσίου) ή από • Συνδυασμό αυτών των παραγόντων. ΘΕΡΑΠΕΙΑ – ΑΝΤΙΜΕΤΩΠΙΣΗ Η αντιμετώπιση των κάλων, όπως κάθε πάθηση, απαιτεί τις γνώσεις ενός ειδικού Ποδιάτρου – Ποδολόγου και προϋποθέτει την τήρηση των κανόνων υγιεινής για την αποφυγή οποιασδήποτε μόλυνσης ή τραυματισμού. Ο ρόλος του Ποδιάτρου - Ποδολόγου είναι να συντηρεί το δέρμα υγιές στα σημεία όπου υπάρχει προδιάθεση να δημιουργηθούν συμπτώματα υψηλής πίεσης και να μειώσει τις αιτίες που δύνανται να προκαλέσουν τις πιέσεις αυτές Οι κάλλοι διακρίινονται σε : Σκληροί ή δακτυλικοί κάλλοι ή τύλοι (hard or digital corn) είναι ξηρές κερατινικές μάζες που συνήθως βρίσκονται στη ραχιαία επιφάνεια των δακτύλων, στην άκρη των δακτύλων καθώς επίσης και στην επιφάνεια του πέλματος του ποδιού. Είναι επιπτώσεις που δημιουργούνται από πιέσεις σε συγκεκριμένο σημείο για μεγάλο χρονικό διάστημα. Μαλακοί κάλοι ή τύλοι (soft or interdigital corn) είναι πολύ επώδυνοι και βρίσκονται μόνο ανάμεσα στα δάκτυλα. Απορροφούν υγρασία από τον ιδρώτα και έτσι μουσκεμένοι δευτεροπαθώς επιμολύνονται από μύκητες και βακτηρίδια. Είναι πολύ συχνοί μεταξύ 4ου και 5ου δακτύλου και μερικές φορές βρίσκουμε δύο βλάβες, τη μία απέναντι από την άλλη (kissing lesions). Η μορφή του κάλου είναι άσπρη και μαλακή επειδή υπάρχει χρόνια διαβροχή (μούλιασμα, υγρασία). Πελματιαίοι κάλλοι ή τύλοι (plantar corn or focal intractable plantar keratosis) είναι μικρές, καλά εντοπισμένες και επώδυνες βλάβες (κάτω από το κνημιαίο σησαμοειδές ή από οξεία προβολή του περονιαίου κονδύλου μιας μετατάρσιας κεφαλής) και περιέχουν κεντρικό κερατινικό βύσμα. Η πρόληψη είναι το μεγαλύτερο βήμα στην αντιμετώπιση και συνέπεια της μείωσης των υψηλών μηχανικών πιέσεων που δέχεται το πόδι. Για το λόγο αυτό : 1) Τα παπούτσια πρέπει να είναι άνετα και να ταιριάζουν απόλυτα στο σχήμα του ποδιού. Επίσης πρέπει να έχουν το σωστό πλάτος και περιθώριο ως και 1,5 cm μεταξύ του μακρύτερου δαχτύλου και της μύτης του παπουτσιού 2) Προτιμότερα είναι τα παπούτσια με χαμηλά και φαρδιά τακούνια που διανέμουν σωστά το βάρος. Επίσης πρέπει να υπάρχει ο απαραίτητος χώρος, το απαραίτητο ύψος και πλάτος στο εσωτερικό του υποδήματος για να μπορεί να χωρέσει το πόδι. Τα υποδήματα να είναι φτιαγμένα από μαλακό δέρμα έτσι ώστε να μην εξασκούνται πιέσεις σε προεξέχοντα σημεία 3) Αποφόρτιση των συγκεκριμένων σημείων : • κατόπιν πελματογραφήματος για την καταγραφή πιέσεων και την κατασκευή ειδικού ορθοπεδικού βοηθήματος προκειμένου να εξισορροπηθούν οι πιέσεις στο πελμα. • Ή με ειδικά προστατευτικά (φτιαγμένα από τον ποδίατρο) 4) Καθημερινή περιποίηση της περιοχής με τρίψιμο με ελαφρόπετα ή ράσπα (λίμα με γυαλόχαρτο) και ενυδάτωση με ειδική κρέμα που μαλακώνει τις σκληρύνσεις και διατηρεί τη φυσική ελαστικότητα του δέρματος Τέλος, να αποφεύγονται τα καυστικά υγρά για τους κάλους (επικάλια), διότι είναι πολύ επικίνδυνα και συχνά δημιουργούν φλεγμονές.
