Endometriosis Pain Diary - PDF Digital Download I created this pain diary during bad flare-ups of my endometriosis this year. I found it was helpful to track where the pain was occurring, the strength and duration of the pain, and what other symptoms I was having. I could then report these symptoms accurately to my doctor. I found diet and exercise was connected with my severity of my symptoms, so I also started tracking what I ate, to see if there was anything that triggered or worsened the pain (in my case dairy and gluten), and whether exercise helped.
