THIS IS A DIGITAL FILE AND YOU WILL NOT RECEIVE A PHYSICAL COPY. IT IS A PDF INTENDED TO BE PRINTED AND WRITTEN ONTO, BUT TEXT CAN BE APPLIED ON SOME PDF EDITING PROGRAMMES OR WEBSITES. I have only tried PFDescape.com and have included instructions for how I use this in the images. Attractively and practically laid out, this worksheet allows you to record Unmet Core Childhood Needs, Schemas, and Modes as you assess a client or as they come up in therapy. The Schema section's labels and colours correspond with the labels on the bottom right of the schema cards, but the document can of course be used without the cards if you don't have them and know your Schema Therapy! Please feel free to print this file as often as you need and to provide print-outs to your clients, but please do not circulate or share the digital file. The file is an A4 document so if this is too big or small for your purposes, please change your print settings to print it at whatever size works best for you! I would recommend printing without borders to give yourself as much space as possible, and I usually choose to "fit to page".
