Use this helpful articulation assessment or speech screening instrument for all phonemes, including blend sounds and voiced and voiceless /th/, /y/, /w/, and /h/ are also included, to help you determine if a child needs further speech assessment or to determine which speech phonemes to work on next. This assessment only takes a few minutes to administer! What's included? Articulation Screening Therapist Form Articulation visual cards Speech Error Sound Inventory (includes sound acquisition ages) Screening Word List Key (shows all the words and what position they are being assessed in) Speech Screening Student List I have included a form that will allow you to keep up with the students you need to screen and a speech error sound inventory. The speech inventory list will allow you to look at the errors made in which position, and you can use the rubric to help you decide if the error sounds are age-appropriate or not. I have laminated the color visual form that I use so that they will last a long time. The target phonemes are in italics and underlined on the therapist form. All phonemes and one of the following blends are included: gl/sn/cr, voiced, and voiceless /th/. You will also find the older version of this assessment included. It does not have the corresponding numbers in the evaluation or visuals. (In case you find the numbers distracting).

Are you looking for a quick and comprehensive all-in-one Speech Therapy Screening with an articulation screener and grade-level language screener on the same form for 2-year-olds through 12th grade? This speech-language screener has an intensive articulation assessment containing all speech sounds that can easily double as an informal assessment to help you develop your articulation IEP goals. Attention: This product is exclusively designed for use by PROFESSIONAL SPEECH PATHOLOGISTS. NOTE: This is a combination of the following two products: Articulation Screening Assessment and a partial combination of the Speech Language Screenings but WITHOUT the quick articulation screening. The grade levels are broken down like this: 2 years 3K 4K 5K 1st 2nd-3rd 4th-5th 6th-8th 9-12th Includes visuals needed to use with the lower level screenings (K3-1st--there's a key on the bottom of each page telling you which visual aids you will need) in the following areas: Color identification Shape Identification Object Identification Concepts Categories Body Parts Articulation Assessment Visuals Plus, you have an articulation screening instrument for all phonemes, including blend sounds on the same form! This assessment only takes a few minutes to administer! The student visuals for the articulation portion are in black and white or color. You can choose to show the students the visuals with or without words. I have laminated the color visual form that I use so that they will last a long time. You could also hand out the black and white form and have the student stamp or color each picture as they say it. The target phonemes are in italics and underlined on the therapist form. All phonemes and BLENDS are included. A form is included so that you can document the students who need a speech screening, and it allows you to document if you tried to pull them but couldn't. I have also included a *Error Sound Inventory* so that you can document what sounds the student has misarticulated and look at the age of acquisition chart on the form to see in what order you should choose sounds and write your goals. I recommend further assessment if they score an 80 or below (on the language portion), but use your clinical judgment. Regarding the articulation portion, I use the error sound inventory and compare the sounds they misarticulated to those they should have at their age to determine if further assessment is needed.
This quick articulation screener is a fast and easy articulation screening tool. Easily assess all phonemes in all positions! Use it to determine whether further evaluation is necessary, or as a baseline/ progress monitoring tool! I've also included a typical development chart to easily assess whether errors are developmental. Use this screener to determine if the speech errors the child says are developmentally appropriate or delayed. This screener only takes about 5 minutes to administer. Some pictures are used for 2 different sounds (e.x., "soap" - initial /s/ and final /p/), less pictures equals quicker screening! This screener uses simplified vocabulary words that most children should be able to label. I carefully selected the words so that each word is "easy" to produce (e.x., there are no words with both a /t/ and a /k/, and the early developing words do not contain blends). Included in this screening packet: Stimulus pictures 2 variations of screening forms - includes phonetic transcription for quick scoring Summary Report The following sounds are assessed: /p, b, m, n, h, w, t, d,. k, g, f, v, j, l, s, z, r/ "sh", "ch", "j", "th", /s/ blends. /l/ blends, /r/ blends, and multisyllabic words I always appreciate feedback! Feel free to give me suggestions on how I can alter this screener to suit your needs better! Leaving feedback also earns you "credits" which can be used for discounts on future purchases. (To leave feedback, just go to your My Purchases page. Next to each listed purchase will be a Provide Feedback button. Click it and you'll be taken to the page to leave a rating and short comment) Thanks!

How to streamline your speech and language evaluations by starting with a PICO question and form a detailed plan for assessment.
With the beginning of every new school year comes the beginning of screening season. Requests from teachers and parents to have their child's speech and language abilities quickly assessed come pouring in, or at least that can be how it feels. One of the most common areas I get requests for is listening comprehension. This request typically stems from the following complaints: - Difficulty answering questions from text or - Difficulty understanding new vocabulary introduced from text - Unable to create inferences - Difficulty predicting - Unable to summarize information - etc. Yes, difficulty following directions is often included in

A must-have for any speech language pathologist performing kindergarten screenings! Now with a No-Print Option included! Screen, Score and Assess! I created this comprehensive toolkit because I needed a thorough Kindergarten Screening for assessing my students and determining “next steps” in their intervention and RtI process. Often, I would screen a student but then require additional information in a particular area before determining intervention needs. This toolkit contains real-life photos that are easily relatable. Cut-off scores determine whether a student requires additional testing in that particular area. This is the ENGLISH version of the Preschool Screening toolkit. For the Spanish Version, click HERE. Looking to screen and assess more grades? Check out my BUNDLE Toolkit that is discounted over 30%! In this packet you will find: ✱ A comprehensive Screening Tool to screen the areas of: ✓Comprehension for Reading/Writing ✓ Phonological Awareness ✓ Vocabulary ✓Grammar ✓ Articulation ✱Student handouts to go along with the Screening Tool ✱Answer and Scoring Guide for the SLP ✱No Print Screening Option for Screening to use on tablet or computer ✱A comprehensive “Next Step” Assessment Tool to assess the areas of: ✓Comprehension for Reading/Writing ✓Phonological Awareness ✓Vocabulary ✓Grammar ✓Articulation ✱Parent letters and permission to screen, assess and intervene In addition, I’ve included student lists to keep track of all your screenings and data A few notes: • This toolkit was created for Speech Language Pathologists. SLPs have the most training when it comes to assessing areas of language and articulation so I geared this packet to them. • These are not norm-referenced testing measures. Although they contain scoring guidelines and “loosely” created cut-off scores, they rely on your clinical judgment to determine whether a student could benefit from addition intervention. *Now offering discount bulk licenses (districts buying more than 10 individual licenses)! Please email me at [email protected] to find out if your district qualifies and for more information.
As part of every evaluation, re-evaluation, assessment or screening, it is ALWAYS imperative to see what the students will share with you in terms of their own impressions of their speech language and social communication skills. This Form is my Go-To when I want to give my students a quick and efficient self-rating assessment, when I want them to reflect on their own speech language and social communication skills, or when I want to see where they are at in terms of their feelings about their overall language abilities. The 17 questions touch on areas such as articulation, fluency/stuttering, executive functioning, dyslexia, as well as the emotional aspect of communication (social skills, social anxiety- selective mutism, self confidence) and knowledge of foreign language/bilingualism. Tip: At the beginning of the year, when I set up interactive notebooks with my students, they fill out this form and glue it to the first page of the notebook. This helps us reflect on their current skills and helps them come up with an idea of what else they would still want to work on.

Stuttering Assessment: How to do a Fluency Evaluation Stuttering, the most common fluency disorder, is an interruption in the flow of speaking characterized by specific types of disfluencies, including repetitions of sounds, syllables, and monosyllabic words (e.g., “Look at the b-b-baby,” “Let’s go out-out-out”); prolongations of consonants when it isn’t for emphasis (e.g., “Ssssssssometimes we stay home”);
