Abdominal wall pain is often mistaken for intra-abdominal visceral pain, resulting in expensive and unnecessary laboratory tests, imaging studies, consultations, and invasive procedures. Those evaluations generally are nondiagnostic, and lingering pain can become frustrating to the patient and clinician. Common causes of abdominal wall pain include nerve entrapment, hernia, and surgical or procedural complications. Anterior cutaneous nerve entrapment syndrome is the most common and frequently missed type of abdominal wall pain. This condition typically presents with acute or chronic localized pain at the lateral edge of the rectus abdominis that worsens with position changes or increased abdominal muscle tension. Abdominal wall pain should be suspected in patients with no symptoms or signs of visceral etiology and a localized small tender spot. A positive Carnett test, in which tenderness stays the same or worsens when the patient tenses the abdominal muscles, suggests abdominal wall pain. A local anesthetic injection can confirm the diagnosis when there is 50% postprocedural pain improvement. Point-of-care ultrasonography may help rule out other abdominal wall pathologies and guide injections. The management of abdominal wall pain depends on the etiology. Reassurance and patient education can be helpful. Local injection with an anesthetic and a corticosteroid is an effective treatment for anterior cutaneous nerve entrapment syndrome, with an overall response rate of 70% to 99%. For refractory cases that require more than two injections, surgical neurectomy generally resolves the pain.

Bell's palsy is a peripheral palsy of the facial nerve that results in muscle weakness on one side of the face. Affected patients develop unilateral facial paralysis over one to three days with forehead involvement and no other neurologic abnormalities. Symptoms typically peak in the first week and then gradually resolve over three weeks to three months. Bell's palsy is more common in patients with diabetes, and although it can affect persons of any age, incidence peaks in the 40s. Bell's palsy has been traditionally defined as idiopathic; however, one possible etiology is infection with herpes simplex virus type 1. Laboratory evaluation, when indicated by history or risk factors, may include testing for diabetes mellitus and Lyme disease. A common short-term complication of Bell's palsy is incomplete eyelid closure with resultant dry eye. A less common long-term complication is permanent facial weakness with muscle contractures. Approximately 70 to 80 percent of patients will recover spontaneously; however, treatment with a seven-day course of acyclovir or valacyclovir and a tapering course of prednisone, initiated within three days of the onset of symptoms, is recommended to reduce the time to full recovery and increase the likelihood of complete recuperation.

Patients with generalized anxiety disorder experience worry or anxiety and a number of physical and psychologic symptoms. The disorder is frequently difficult to diagnose because of the variety of presentations and the common occurrence of comorbid medical or psychiatric conditions. The lifetime prevalence is approximately 4 to 6 percent in the general population and is more common in women than in men. It is often chronic, and patients with this disorder are more likely to be seen by family physicians than by psychiatrists. Treatment consists of pharmacotherapy and various forms of psychotherapy. The benzodiazepines are used for short-term treatment, but because of the frequently chronic nature of generalized anxiety disorder, they may need to be continued for months to years. Buspirone and antidepressants are also used for the pharmacologic management of patients with generalized anxiety disorder. Patients must receive an appropriate pharmacologic trial with dosage titrated to optimal levels as judged by the control of symptoms and the tolerance of side effects. Psychiatric consultation should be considered for patients who do not respond to an appropriate trial of pharmacotherapy.

By Dr. Meera Shah What is IUI? IUI (intrauterine insemination) is a procedure in which concentrated sperm is placed into the uterine cavity at the time of ovulation. It is usually a quick, safe, and painless procedure that involves placing a speculum to visualize the cervix, and then inserting a small catheter through the cervical canal into the upper uterus, where the sperm can swim into the fallopian tube and find the ovulated egg. This procedure can be performed by a physician (fertility specialist or OBGYN) or a mid-level provider (ie. nurse practitioner or physician assistant). Who is a good candidate for IUI? IUI is recommended for the following indications: Unexplained infertility Male-factor infertility (low sperm count or motility) Lesbian couples desiring pregnancy with donor sperm Single women desiring pregnancy with donor sperm Couples unable to have penetrative intercourse due to conditions such as vaginismus (pain during sex) or erectile dysfunction Women with cervical stenosis (scarring or closure of the cervix due to prior medical procedures) In order for an IUI to be successful, the fallopian tubes must be open and healthy. Some women who have had a history of pelvic infections, endometriosis, or abdominal surgery may be at risk for tubal damage, therefore it is important to first evaluate the tubes using tests such as a hysterosalpingogram (HSG) or saline sonohystersterogram (SHG). 🌟 Shop Fertility Products ⟶ How common is IUI? IUI is a very common procedure and is usually the first-line treatment for couples with infertility. Because it less invasive, cheaper, and ‘more natural,’ many couples opt to try IUI prior to in-vitro fertilization (IVF). What is the success rate of IUI? The major downside of IUI is the lower success rate. The success rates range from eight to fifteen percent per cycle and depend on a number of factors, including the indication, age of the woman, and whether IUI is combined with hormone treatments to increase the number of eggs. The cumulative success rate over the first three cycles may be as high as 30-40%. How much does it usually cost? The cost of one IUI cycle can range from $500-$4,000, depending on if there are any additional medications or monitoring required for the treatment. How many cycles do people typically do before getting pregnant? In general, three to six cycles of IUI are recommended before IVF. This is based on the decreasing efficacy and cost-effectiveness of IUI after each subsequent cycle. What should I do to prepare for IUI? Talk to your doctor about whether IUI is right for you. There are many factors that are involved in making a recommendation, including your age, ovarian reserve, semen analysis results, and most importantly, your overall family planning goals. Postpartum hemorrhage, the loss of more than 500 mL of blood after delivery, occurs in up to 18 percent of births and is the most common maternal morbidity in developed countries. Although risk factors and preventive strategies are dearly documented, not all cases are expected or avoidable. Uterine atony is responsible for most cases and can be managed with uterine massage in conjunction with oxytocin, prostaglandins, and ergot alkaloids. Retained placenta is a less common cause and requires examination of the placenta, exploration of the uterine cavity, and manual removal of retained tissue. Rarely, an invasive placenta causes postpartum hemorrhage and may require surgical management. Traumatic causes include lacerations, uterine rupture, and uterine inversion. Coagulopathies require dotting factor replacement for the identified deficiency. Early recognition, systematic evaluation and treatment, and prompt fluid resuscitation minimize the potentially serious outcomes associated with postpartum hemorrhage.
