A 35-year-old woman was started on a second course of isotretinoin for recurrent acne. Her previous treatment was two years earlier and was uneventful. A few weeks after restarting the medication, at a dose of 0.5 mg per kilogram per day, she experienced some malaise, muscle aches, and mild joint pain. There were no new skin findings. She called me and I allowed that I had not heard of these symptoms with isotretinoin, but ordered a CBC and an ANA. The CBC was normal, but the ANA was positive at 1:640 with a homogeneous pattern. A PubMed search retrieved no references to drug-induced lupus and isotretinoin. However, a Google search found some anecdotal reports and a few cases of suspected DIL with isotretinoin (there were 5 alleged cases of DIL out of 27,831 self- reported isotretinoin side-effects) eHealthme. With regards to our patient, I am going to repeat her ANA and obtain an anti-histone antibody test. If the ANA is still positive I will have her stop the isotretinoin and repeat the lab studies after 2–3 weeks. Should this likely be DIL then I think it is important that there be a report in the medical literature as undoubtedly other patients will experience this.

A 50-year-old female with a past medical history of bone tuberculosis diagnosed nine months ago was admitted in our infirmary for persistent fever with no evident cause. The patient was treated with isoniazid, rifampicin, pyrazinamide, and ethambutol for seven months and for the past two months, she was taking isoniazid and rifampicin. She went to our emergency room (ER) for back pain and fever that she had been experiencing for the last month. She was admitted with suspicion of disseminated tuberculosis that was never confirmed. Physical examination was unremarkable. Blood tests showed an elevation of inflammation parameters. A computed tomography (CT) scan of the chest showed a mild pleural effusion. She remained with fever during the three weeks in the infirmary while undergoing many other studies that were all negative. The back pain would change sides, and three consecutive thoracic radiographies showed a small-sized pleural effusion that was either predominantly right-sided or left-sided. Several differential diagnoses were considered in the process, namely an active infection, neoplasia, or autoimmune disease. The search for circulating lupus anticoagulant was positive. Antinuclear antibodies (ANA) were positive and the anti-histone antibody was strongly positive. At this point, we suspected a drug-induced lupus diagnosis, and isoniazid was discontinued. Following discontinuation of isoniazid, back pain and fever subsided and patient was discharged after one week. This case is a diagnostic challenge because of the rarity and symptom severity of isoniazid-induced lupus. Isoniazid rarely induced this lupus-like syndrome, with an incidence of considerably less than 1%.

Vasculitis, lupus, sarcoidosis and other disorders all may involve the brain and/or spinal cord, and in each instance, presentation with exclusively neurological features is far from unknown. In most such disorders, there are no diagnostic clinical features; many of these diseases mimic each other. Once suspected, it is commonly the case that no tests are available that categorically prove the disease to be present—other than cerebral biopsy, and even this has limited sensitivity and clearly carries a certain risk. A number of investigations often considered valuable are neither sensitive nor specific, and are prone to over-interpretation. CNS inflammatory diseases may be aggressive, seriously disabling or even fatal—and yet most are highly treatable. Clinical nomenclature and definitions have not helped a complex field—many are at best confusing, and often wholly misleading. Vasculitis, for example, is neither a diagnosis nor a disease, but rather a histopathological description (intramural inflammation, often but not invariably with additional perivascular infiltrates, accompanied by necrosis of the blood vessel wall). Lupoid sclerosis, a term firmly embedded in the literature, probably does not exist; lupus vasculitis is a ‘diagnosis' in rather commonly usage, but in fact very rarely is it a useful or accurate description of CNS lupus-related disease. Approaches to confirming CNS inflammatory diagnoses can be suggested, with increasing recognition of the limitations of investigative techniques. Finally, and admittedly again almost wholly informed by clinical research in systemic inflammatory disorders, rather than good quality clinical trials in patients with neurological disease, treatment regimes are improving.
