Στα πλαίσια της κατασκηνωτικής μου εμπειρίας, το περασμένο καλοκαίρι, είχα την τύχη να γνωρίσω ένα κορίτσι, λίγο διαφορετικό από όλα τ’ άλλα. Συζητώντας με την εκπαιδευτικό που τύχαινε να συνοδεύει το κορίτσι στην εν λόγω κατασκήνωση, για άτομα με ειδικές ανάγκες, αναφερθήκαμε στο ιδιαίτερο σύνδρομο που αυτό είχε, γνωστό κι ως Σύνδρομο Angelman. Σκέφτηκα λοιπόν, με αφορμή, αυτήν μου τη γνωριμία, το παρόν άρθρο, να αναφέρεται ακριβώς σε αυτό το σύνδρομο... Το σύνδρομο Angelman.... Θέλοντας μάλιστα να της το αφιερώσω. Συγκεκριμένα, στόχος του άρθρου είναι να προσφέρει μια σύντομη αναφορά στο νευροψυχολογικό προφίλ, σε ορισμένα από τα χαρακτηριστικά, καθώς και σε ορισμένους τρόπους παρέμβασης για τα παιδιά με σύνδρομο Angelman. Ας ξεκινήσουμε! Το Σύνδρομο Angelman αποτελεί μια νεύρο-αναπτυξιακή διαταραχή, η οποία επηρεάζει 1 στις 20.000 γεννήσεις, δηλαδή σε 20.000 γεννήσεις το 1 παιδί θα γεννηθεί με σύνδρομο Angelman. Το σύνδρομο προκαλείται από μία ποικιλία γενετικών ανωμαλιών, στη περιοχή του χρωμοσώματος 15, η οποία οφείλεται σε γονιδιακή αποτύπωση. Κατά τον πρώτο χρόνο της ζωής τους, τα περισσότερα παιδιά με αυτό το σύνδρομο παρουσιάζουν καθυστέρηση στην κατάκτηση των βασικών σταδίων της ανάπτυξης, καθώς και καθυστέρηση στην ανάπτυξη της περιφέρειας της κεφαλής. Έτσι, τα παιδιά με σύνδρομο Angelman, τείνουν να έχουν ένα δυσμορφικό προσωπείο, το οποίο περιλαμβάνει μικρό μέγεθος κεφαλής, μεγάλο στόμα, μεγάλο κενό ανάμεσα στα δόντια, προτεταμένο πιγούνι, έντονο προγναθισμό και προεξέχουσα γλώσσα. Το σύνδρομο Angelman, χαρακτηρίζεται από βαριάς μορφής μαθησιακές δυσκολίες, νοητική υστέρηση και συχνά κρίσεις επιληπτικού τύπου. Παρουσιάζει, επίσης, διαταραχές στην κίνηση (π.χ. αταξία). Αυτό συμβαίνει, διότι, παιδιά με το σύνδρομο αυτό καταφέρνουν να καθίσουν στους 12 μήνες της ανάπτυξης τους και να περπατήσουν γύρω στα 3 με 4 έτη. Ένα 10% των περιπτώσεων, μάλιστα, δεν καταφέρνει να περπατήσει. Διαταραχές επίσης, εμφανίζονται και στον λόγο του παιδιού. Στην πλειονότητα των περιπτώσεων συγκεκριμένα, δεν αναπτύσσεται καθόλου λόγος (λίγες ή και καθόλου λέξεις). Αν και τα παιδιά αυτά έχουν προβλήματα ομιλίας, έχουν, ωστόσο, έντονη επικοινωνιακή ικανότητα και δείχνουν την αγάπη τους πολύ έντονα σε όποιον ενδιαφέρεται γι’ αυτά. Τα άτομα με σύνδρομο Angelman, είναι πολύ στοργικά. Παρουσιάζουν, δηλαδή, ένα ιδιαίτερο φαινότυπο συμπεριφοράς, το οποίο περιλαμβάνει σπασμωδικές και πολύ απότομες κινήσεις, συχνό γέλιο (θυμάμαι χαρακτηριστικά εκείνο το μεγάλο χαμογελαστό προσωπάκι της, γεμάτο όρεξη για παιχνίδι και χορό καθώς και το βάδισμα μαριονέτας που είχε!), ιδιαίτερη συμπάθεια στο νερό και συχνά διαταραχές στον ύπνο. Η συμπεριφορά τους, τις περισσότερες φορές(σχεδόν πάντα!) ενέχει χαρά και ενθουσιασμό, η οποία μάλιστα πολλές φορές είναι και χωρίς κάποιον εμφανή λόγο. Παρουσιάζουν, επίσης, υπερκινητικότητα, χωρίς να υπάρχουν, ωστόσο, ενδείξεις επιθετικότητας, ελλειμματική προσοχή, και μειωμένη ανάγκη για ύπνο. Το προσδόκιμο επιβίωσης των πασχόντων δεν διαφοροποιείται από αυτό του γενικού πληθυσμού. Το μόνο ίσως πρόβλημα έγκειται στο ότι πιθανότατα δε θα μπορέσουν ποτέ να εξελιχθούν σε ανεξάρτητα άτομα. Υπάρχουν, όμως, ορισμένοι τρόποι με τους οποίους μπορούμε να μειώσουμε την ένταση των παραπάνω συμπτωμάτων. Αρχικά, στον τομέα της εκπαίδευσης, κρίνεται απαραίτητη η αξιολόγηση των βασικών δυνατοτήτων και αδυναμιών των παιδιών αυτών, ώστε να υπάρξει και η κατάλληλη υποστήριξη από μέρους των εκπαιδευτικών, μέσα από τη διαμόρφωση ενός κατάλληλα προσαρμοσμένου, με βάση τις ανάγκες πάντοτε του παιδιού, σχολικού περιβάλλοντος. Ένα πρώιμο, ενεργητικό και εξατομικευμένο πρόγραμμα παρέμβασης θα ήταν το ιδανικότερο. Από την άλλη, για την βελτίωση της λεπτής κινητικότητας των παιδιών με σύνδρομο Angelman, θα ήταν χρήσιμη η παραπομπή του παιδιού σε έναν εργοθεραπευτή, ενώ για τη βελτίωση της αδρής κινητικότητας σε έναν φυσικοθεραπευτή. Ο τελευταίος θα δώσει αρκετές συμβουλές σχετικά με την ισορροπία, τη θέση και τη διαχείριση των μυϊκών συσπάσεων. Για την βελτίωση του λόγου του παιδιού, απαραίτητη κρίνεται η επίσκεψη σε έναν λογοθεραπευτή. Θα ήθελα, σε αυτό το σημείο, να τονίσω, πως αν και δε θεραπεύονται πλήρως όλα τα συμπτώματα του συνδρόμου, οι επιληπτικές κρίσεις μειώνονται σημαντικά με τα χρόνια. Τελειώνοντας, θεωρώ πως για την εκπαιδευτική αντιμετώπιση των δυσκολιών αυτών, σημαντικό ρόλο παίζει η αλλαγή των αντιλήψεων των εκπαιδευτικών, σχετικά με τα σύνδρομα, καθώς και των καθημερινών διδακτικών προσεγγίσεών τους. Μέσα από την ευαισθητοποίηση και την επιμόρφωση των εκπαιδευτικών και των γονέων, ευελπιστούμε να υπάρξει βελτίωση της εκπαιδευτικής πραγματικότητας για τους μαθητές μας με γενετικά σύνδρομα.
